Injector seals, copper washers at the end of each injector, they break and exhaust gases vent up past the seal which in turn blows that injector cover in your photo, looks messy but shouldn't be a big job to repair, just labour costs and a few copper washers.
Hi, looks very like injector seals, had mine done about 2 months ago and like the photo completely destroyed cover, also you may have to replace fuel return pipe as the carbon build up makes them very brittle. Best to replace all four injector seals.
+1 for injector seals. common as you like on many diesel engines. in fact one of the biggest culprits, in the early days of common rail diesels, were Mercedes CDi engines.
